Worker's Compensation Records are available by mail only. A signed release is required. Fee per copy. Search information required: name, date of birth, SSN, date of injury, SASE.

Vital Statistics

Birth and Death records are available. Records are also accessible by computer. For birth records include persons full name, date and place of birth, father's name, mother's maiden name, name of hospital, SSN, and signed release. Birth certificate fee required. All copies are certified. For death certificate, include full name and required fee.
